This is a short-term medical respite program for seriously ill homeless men and women – those individuals who are too sick for a shelter but not sick enough for a hospital. The program provides short-term medical and recuperative services that include dermatology, optometry, HIV related services, dental, and behavioral health testing.
Requires a referral from a social worker, an agency case manager or clinician who has appropriate medical information, or a DOC nurse, or some other medically-knowledgeable professional. Must have a referral from a hospital, shelter, clinic, social worker, or other health care service provider. Patients cannot self-refer for admission. Can be given through walk-in clinic.
